Forms and Information sheets
Forms and information sheets are available in many different file formats so that you can choose the file that best fits your needs. Many are available as Microsoft Word or Excel documents. In addition, there may be two types of portable document format (PDF) file. Regular PDF documents may be viewed and printed. Fill-in PDF documents may be filled out using Adobe Acrobat Reader (free from Adobe) or Adobe Exchange (commercial software) and then printed.
Adobe Acrobat Reader 4.0 and above is recommended. ORSPA PDF forms are currently being tested with Adobe Acrobat and Reader 8.1.1; no changes will be made to the ORSPA forms until the full impact of the change is known.
