Get approval signaturesOn this page: The bottom lineBefore you send your proposal to your SPO to submit to the sponsor, you need to get the appropriate internal approval signatures. The backbone of this routing process is the ORSPA 300A form. Once you have filled out all the necessary forms and obtained all the necessary signatures, you can send the proposal package to your SPO. The ORSPA 300A: Proposal Routing and Approval form (blue sheet)The ORSPA 300A form must be filled out for every proposal that will be sent to an external sponsor. Fill out the parts that apply to you and then get it signed by your department chair/unit director and dean for approval. If you have co-investigators on the project, they will also need to sign along with their department chair/unit directors and deans. Note that the ORSPA 300A will indicate whether you should also fill out the ORSPA 300B, 300C, or 300D. If you have filled out any of the forms below (see the Key processes page), include them with your ORSPA 300A form when getting signatures from your department chair/unit director and dean.
Annual Objectivity in Research QuestionnaireSection K of the ORSPA 300A form says that you must fill out the Annual Objectivity in Research Questionnaire. This is an online questionnaire regarding any financial interests you have outside of the university. The online questionnaire is completely secure and confidential. If you do not complete this form annually, your award will be held until you do so. |
